When you are using "Create a user input form" Wrk Action and get an error saying "Create a user input form caused an error: Form title and/or form description was not successfully updated. If error persists, please contact support.." - this is due to having a newline character in your "Form instructions". If you re-write the "Form instructions" to not include any newline characters, then the Wrk Action will work normally.
